Case Management is a service that assesses your needs, develops a care plan, and coordinates that care to ensure you have access to the support and resources required to meet your unique needs. You will work collaboratively with your case manager to identify priority actions/services needed in your life and create a plan to address those issues first, while also looking toward longer-term goals. Your Case Manager will work with you to monitor your progress, reassess your needs and care plan regularly, and address new challenges as they arise. Case Managers are advocates for their patients, helping you to access and secure resources to eliminate barriers and improve your overall health and well-being.
**Case Management services are currently available in our Kentucky, Ohio, West Virginia, and Virginia locations.
How Can a Case Manager Help You?
Case Managers assist you with any area of life, both large and small, in which you are experiencing barriers or need assistance. These include, but are not limited to, the items below. You should speak to your case manager about anything that comes up where you may need support, assistance, or guidance.
- Basic needs: food, clothing, utilities, cell phone
- Obtaining personal documents
- Applying for public benefits: Medicaid, food stamps, etc.
- Housing & related needs: furniture, appliances, etc.
- Transportation
- Education
- Employment, resume building and interview skills
- Budgeting & money management
- Mental health, medical & dental care
- Legal support & referrals
- Linkages to support groups
When referring you to a service or program, your Case Manager can help you by contacting them on your behalf, contacting them together and/or attending the first meeting to ensure you receive the best, most appropriate service and are comfortable throughout the process.
Who is eligible for Case Management?
Any AppleGate Recovery patient is eligible for Case Management. This is a valuable service that is a part of our comprehensive treatment programs for those recovering from a substance use disorder.
How much does this service cost?
Most accepted insurance plans cover Case Management services, but they will be subject to any co-pay or coinsurance requirements of your plan. As a part of the comprehensive treatment program offered to self-pay patients, case management and counseling services are included in the monthly patient fee.
