Eligibility
Before your intake appointment, an AppleGate Recovery staff member will conduct an eligibility assessment to determine whether office-based opioid treatment (OBOT) and using medication-assisted treatment (MAT) is right for you. The current requirements include:
- Patients must be 18 years of age or older
- Valid photo ID
- Evidence of opioid addiction
- Agreement to terms and program policies
Intake Process
After you set up an intake appointment by calling ahead and completing an eligibility assessment, you are typically able to see a medical provider within 24-48 hours. During the 2-3 hour intake process, AppleGate Recovery staff members will make you feel as comfortable as possible while you talk about your treatment plan together in a judgment-free environment and encourage you to ask questions and discuss your concerns relating to your medication-assisted treatment (MAT) plan.
Medical Assessment
Once you complete the initial paperwork, you will meet with your medical provider to review your medical history with you and complete a physical assessment as part of the intake process. Your medical provider will then determine which medication and dosage is best suited for your individual needs. Once admitted to the program and upon completion of the assessment, your medical provider will write a prescription that you can have filled at chosen local pharmacy of your choice.
AppleGate Recovery medical providers are certified members of the American Society of Addiction Medicine dedicated to increasing access to and improving the quality of substance use disorder treatment. They are licensed and trained to prescribe buprenorphine, Suboxone and Zubsolv based on what is most appropriate for each patient. To learn more about each of these medications, click here.
Beginning MAT
Once you are fully admitted into the AppleGate Recovery treatment program, and your medical provider has supplied you with a prescription that matches your individual needs, you will then work with our administrative staff to set up a substance use counseling appointment offered at selected locations or we will give you a referral to a nearby counselor. After your appointment is set, we will set you up with a follow-up appointment to see how you are progressing with your medication.
MAT works best when combined with individual and group counseling sessions where you can discuss your thoughts and progress through recovery.
Follow-Up Appointment
Follow-up appointments allow you and our medical providers to discuss how your treatment is progressing. The appointments consist of a brief medical examination, medication check, and concerns about dosage, side effects and other health concerns. Follow-ups can be scheduled based on your specific needs and the medical provider’s recommendation.
Substance Use Counseling
AppleGate Recovery focuses on treating the whole patient while enrolled in our office-based opioid (OBOT) treatment programs. Combining medication-assisted treatment with substance use counseling has shown promising evidence of successful, long-lasting recovery. AppleGate Recovery offers in-house individual and group substance use counseling at select locations, while other clinics provide you with referrals to outside counselors.
